Hours are 8:00 a.m. - 5:00 p.m., Monday - Friday. The office is closed weekends and holidays
Please call us within 24 hours of an appointment if you must cancel or reschedule so we may serve other patients' needs efficiently.

| Previous test results and copies of x-rays/imaging studies | |
| List of current medications, including over-the-counter & herbal medications; Be prepared to provide information on current dosage amounts and renewal needs | |
| Medical forms to be completed by the health care provider | |
| Blue U-M Registration Card | |

| List of written questions you have | |
| A referral, if required |
