Transitional Care Clinic
The Transitional Care Clinic provides service to older patients who are making the transition from hospital to home or from sub-acute care to home. Patients who have had acute hospitalizations, complex emergency room visits or sub-acute care stays and who require urgent follow-up care are appropriate for this clinic. This clinic specializes in patients with multiple medical problems or those who may need short-or long-term placement to prevent re-hospitalization. Medical care and social work support will be provided for the patient and family.
