Diabetes

Diabetes is a chronic illness that requires ongoing medical care and self-management education. People with diabetes do better when they assume an active role in their care. The management plan should be developed by the medical team and the patient together. The plan should consider the person's age, schedule, physical activity, eating patterns, social situation and personality, cultural factors, and other medical conditions. There is excellent clinical evidence supporting the importance of good diabetes control and how this affects the current and future health of people living with diabetes.
