Obtaining Your Radiology Exams on CD
Pick Up BY Patient
Patient CDs can be picked up from 6:00 a.m. - 9:00 p.m. at the University Hospital, B1-D240. Please call ahead at 734-936-4516 to allow time for the images to be prepared. To arrange pick up of images outside of these hours, please call 734-936-4516. CDs are only released to the patient.
Pick Up For Patient
An authorization signed by the patient (or the legally authorized representative of the patient) is required for anyone other than the patient to pick up CDs or any medical information. Please fill out the Authorization to Release Patient Information Form (PDF) and bring completed form to the University Hospital, B1-D240. Sending Exams to Another Provider or Institution
Patients wishing to have their radiology exams sent to another provider or institution can fill out and fax the Authorization to Release Patient Information Form (PDF) to 734-936-8280.
