Suggested Pre-Referral Evaluation and Management Guidelines
This includes post amputation, post thoracotomy, post mastectomy, post herniorraphy and other post operative pain syndrome. A useful set of criteria to define post op chronic pain syndrome is:
pain that develops after surgical procedure
pain of at least two months duration
exclusion of other causes of the pain
exclusion of preexisting pain
Trial of anticonvulsant agents, and/or TCA's.

Relevant Medical History
Imaging Reports (X-Ray, CT, MRI)
Referral to U of M Back & Pain Center. Interventional options include nerve blocks, pulsed RF of the dorsal root ganglion, cryoablation, and spinal cord stimulation.
