Prior myocardial infarction
Suggested Pre-Referral Evaluation and Management Guidelines
1)  Exam for vascular disease, cutaneous xanthoma
2)  CBC, Diff, UA, Basic, Lipids, Hs-CRP
3)  ECG
4)  Chest x-ray
Suggested Additional Test/Management Prior to Specialty Visit Exercise stress test for risk stratification; esp. if not done previously
Patient Education/Information (includes preps)  
Appointment Timeframe 2-6 weeks
