Suggested Pre-Referral Evaluation and Management Guidelines
Continue to treat blood pressure consider adding additional agents.
Suggested Additional Test/Management Prior to Specialty Visit
1)
Renal Ultrasound w/ dopplers.
2)
UUA, Renal Panel (Glu, UN, Creat, Sod, Pot, Cl, CO2, Cal, Phos, Alb). Quantitate proteinuria with spot protein:creatinine ratio.
Patient Education/Information (includes preps)
High Blood Pressure
Appointment Timeframe
If renal function is stable and PCP is managing blood pressure, this is not urgent (30 days).
