Dementia/Cognitive Concerns

Suggested Pre-Referral Evaluation and Management Guidelines Consider depression, delirium, and medication effects. Assess living situation (medication compliance, financial management, driving safety, nutrition, behavior, caregiver stress).
Are there focal neurological findings, a gait disturbance, or rapidly progressive symptoms?
Suggested Additional Test/Management Prior to Specialty Visit Routine dementia evaluation: CBC, TSH, Vit B12, chemistry profile including renal and liver function tests, brain imaging with CT or MRI (preferred if vascular risk factors present)
Patient Education/Information (includes preps)
1) Inform patient to plan to attend Cognitive Clinic appointment accompanied by a family member or companion who can serve as informant
2) Provide medical records including CD of cranial imaging to bring to appointment (if non-UMHS patient).
Appointment Timeframe Next available Cognitive Disorders New Patient clinic visit. Neuropsychometric testing may be scheduled at time of NP visit.
