| 1) | Screening or dedicated sinus CT without contrast for chronic symptoms without evidence of complications. For patients who are suspected of having complications (progressive swelling around the eye over weeks or months, patients suspected of having intracranial complications, etc) should have dedicated axial and coronal CTs 3mm cuts WITH CONTRAST. UM radiology preferred. If outside imaging obtained, patients MUST hand carry actual or copies of films (or CDs or DVDs) to appointment. |
| 2) | Provide documentation of prior treatment and response to therapy. |
| 3) | Consider sweat test for cystic fibrosis. |
