Suggested Pre-Referral Evaluation and Management Guidelines
Referral is indicated if the patient has:
- Longstanding, chronic active disease and wishes surgery
- High grade dysplasia on biopsy
- Persistent and significant bleeding; anemia
- Disease is refractory to medical management or poorly controlled by aggressive medical treatment
Suggested Additional Test/Management Prior to Specialty Visit
Records should be sent to clinician for review prior to visit.

Colonoscopy within past 6 months- report available at clinic visit

List of all UC-related drugs and dosages within 3 months
If CT, Barium enema or other imaging has been done within past year, please bring images on CD
