Ulcerative colitis inflammatory bowel disease

Suggested Pre-Referral Evaluation and Management Guidelines Referral is indicated if the patient has:
- Longstanding, chronic active disease and wishes surgery
- High grade dysplasia on biopsy
- Persistent and significant bleeding; anemia
- Disease is refractory to medical management or poorly controlled by aggressive medical treatment
Suggested Additional Test/Management Prior to Specialty Visit Records should be sent to clinician for review prior to visit.

- Colonoscopy within past 6 months- report available at clinic visit
- List of all UC-related drugs and dosages within 3 months
If CT, Barium enema or other imaging has been done within past year, please bring images on CD
Patient Education/Information (includes preps)  
Appointment Timeframe Within 4 weeks
How to Get Results to Consultant Any records that are received should be given to consultant to review prior to clinic visit.
