Incidentally Discovered Adrenal Mass
Suggested Pre-Referral Evaluation and Management Guidelines Criteria for referral
  • Patients with abnormal biochemical screening
  • Adrenal mass >2.5 cm
  • Lipid poor adrenal mass (by CT or MRI)
Do not refer:
  • Patients with lipid rich adrenal mass <2.5 cm that have normal biochemical screening
Suggested Additional Test/Management Prior to Specialty Visit Tests required for referral
  • 24 hour urinary cortisol or overnight 1 mg dexamethasone suppression test (normal <1.8 mcg/dL)
  • Plasma or 24 h urine metanephrines
  • If hypertension and/or hypokalemia, obtain plasma renin and aldosterone (cut-off for referral is aldosterone >10 ng/ml AND renin < 1 ng/ml/h)
  • Adrenal CT scan or MRI images (CD) and report must be brought to the visit. Noncontrast CT or adrenal-protocol CT preferred.
