Calcium & Bone: Osteoporosis
Suggested Pre-Referral Evaluation and Management Guidelines

Female: Postmenopausal
Criteria for referral (at least one of the following)

  • T score < -2.5 or -1.0 to -2.5 with history of fragility fracture
  • Failed bisphosphonate treatment
  • Secondary osteoporosis

Female: Premenopausal
Criteria for referral (at least one of the following)

  • Z score < -2.0
  • Z score > -2.0 with history of fragility fracture
  • Secondary osteoporosis (e.g. Amenorrhea, Cushing's syndrome or glucocorticoid use, rheumatoid arthritis, lupus, lactation-induced bone loss with fracture)

Male: History of fragility fracture
Criteria for referral (at least one of the following)

  • T score < -2.5 or -1.0 to -2.5 with history of fragility fracture
  • Failed bisphosphonate treatment
  • Secondary osteoporosis

Male: > 50 years old
(SAME AS ABOVE, History of fragility fracture)

Male: < 50 years old
Same as Female: Premenopausal

Suggested Additional Test/Management Prior to Specialty Visit

Please obtain the following tests:

  • PTH, Intact and Calcium, PTHI (LAB813)
  • Calcium (LAB54)
  • Vitamin D 25 Hydroxy, 25HD (LAB535)
  • Calcium, 24-hour Urine, CALQ (LAB814)
  • Creatinine, 24-hour Urine, CREAQ (LAB712)
  • Comprehensive Metabolic Panel (LAB17)
  • DEXA within past 2 years, NM Bone Density DEXA (RAD3005)
  • For Men: If BMI < 30: 8 AM, Testosterone, TESTO (LAB124)
  • For Men: If BMI >30: 8 AM, Testosterone, Bioavailable, TTBS (LAB839)

External Patients: Hard copies of abnormal test results, ultrasounds, CT scans and MRI's that support the suspected diagnosis are required in order for a patient to be seen. Please fax these, along with the MEND Outpatient Consult Request Form, to: 734-998-1438.

If you are unable to fax information, the patient can hand carry non-UM records to the visit.
