Print This Page     Close Window Consult Request Guidelines for Diabetic Nephropathy     Diabetic Nephropathy Suggested Pre-Referral Evaluation and Management Guidelines 1) Assure recent ophthy eval (nephropathy often found with retinopathy). 2) Control BP to 125/75 if possible & tolerated, often with multiple agents. 3) Consider ACEI/ARB as 1st line, then diuretic and ß-blocker. 4) Assure tight blood sugar control as appropriate. Suggested Additional Test/Management Prior to Specialty Visit 1) Renal Ultrasound 2) Quantify proteinuria with spot protein: creatinine ratio. For patients age >45 years with protein: creatinine > 0.5, spot urine for Bence Jones protein advised.
