Elevated Creatinine - Chronic
Suggested Pre-Referral Evaluation and Management Guidelines
1)
Confirm chronicity (review prior labs)
2)
Refer to nephrology if unclear etiology
3)
Refer to nephrology if associated with proteinuria > 0.5 gm/24 hr or if persistent microscopic hematuria
4)
Refer to nephrology if GFR < 30 ml/min
Suggested Additional Test/Management Prior to Specialty Visit
1)
Renal Ultrasound with dopplers.
2)
UA including micro, spot protein:creatinine ratio
3)
If anemic assess for iron deficiency (ferritin, iron, TIBC).
4)
If iron deficient; consider GI screening as appropriate.

Appointment Timeframe
If renal function is stable and GFR >20, this is not urgent (30 - 60 days); if GFR <20 or decreasing at rate > 1ml/min/month: urgent (< 30 days)
