| Suggested Pre-Referral Evaluation and Management Guidelines |
Urinalysis with microscopy to confirm hematuria. If isolated microscopic hematuria please confirm on 3 occasions.

| Suggested Additional Test/Management Prior to Specialty Visit |
For all patients
1) CBC with platelets and Renal panel
2) Spot urine sodium, calcium, protein and creatinine
3) Urine culture
4) Renal ultrasound
For gross hematuria add PT, PTT

1) For persistent isolated microscopic hematuria: Next available or 4-8 weeks
2) With hypertension, oliguria, rash, join pain, acute renal failure, or Protein/creatinine ratio > 0.5, urgent evaluation required, contact doctor of day through MLINE
