Suggested Pre-Referral Evaluation and Management Guidelines
Ask about history of poliomyelitis and cancer
If symmetrical and proximal, consider myopathy. Review meds.
If malar rash consider dermatomyositis
Suggested Additional Test/Management Prior to Specialty Visit
Refer to PM&R for EMG for neuropathy, motor neuron disease, myopathy (inflammatory vs. non inflammatory)
If muscle disease suspect, obtain CPK, CBC, TSH, T 4, WESR
If on steroids and or statins, consider discontinuing
