Tobacco and nicotine abstinence for at least 6 months. Ambulatory, BMI < 33, no other significant medical disease. Absence of malignancy for at least 5 years(excluding basal cell carcinoma of the skin). No significant coronary artery disease. Suggested Additional Test/Management Prior to Specialty Visit Full pulmonary function testing, 6 minute walk test. High resolution CT of the chest.
