UMHS Patients: See MiChart
External Patients: Should fax the external reports/records to 734-647-8860.
Pathology slides should be sent to UMHS Team 3, CCC Level B1-229, 1500 East Medical Center Drive, Ann Arbor, MI 48109-0913. Slides should arrive 1 week prior to visit. If at all possible, slides should be sent over night via UPS or Federal Express to generate a tracking number.
Films/CDs should be hand carried to appointment. |
