Suggested Pre-Referral Evaluation and Management Guidelines	Refer to Breast Care Center 734 936-6000
Suggested Additional Test/Management Prior to Specialty Visit	Patient must have current imaging (mammogram) prior to appointment and a visit with primary care or ob/gyn physician
Patient Education/Information (includes preps)	For patient information including pre-appointment forms, maps & directions, participating insurance plans, and support: http://www.cancer.med.umich.edu/patients/index.shtml

How to Get Results to Consultant	UMHS patients: See CareWeb External patients fax all outside records to 734 763-1580 Pathology Slides send to: UMHS CCC Floor 1 Rm 1351 1500 E Medical Center Drive Ann Arbor, Michigan 48109-9516 Send UPS or FED-EX. Must arrive before visit Last 3 years of mammograms must be hand carried to appointment and will be reread in our breast Imaging department. This is a separate appointment that we will arrange prior to the visit with us, by mail
